The European Gatekeeper project has now launched its blog where it will feature content written by Gatekeeper’s experts and invited thought leaders on topics such as Web of Things, Artificial Intelligence (AI) in health care, health care data federation, European Health Data spaces, ethics, data protection and trust building, pilot test sites and much more.

A launch that has been carried out with a first article written by our Chief Innovation Officer, Sergio Guillen, who is also the GATEKEEPER Deputy Project Coordinator.

Addressing unmet medical problems in Europe through Artificial Intelligence – Gatekeeper

The purpose of this Blog is not just to talk about project activities and findings, but to create a professional forum to discuss the many related issues. We will focus on the fascinating experiences, new requirements, conditions, demands and new opportunities that will emerge as soon as data driven technologies are incorporated into health care processes. We will also highlight the direct impact produced on the life of older adults and European healthcare systems.

The four main challenges facing Gatekeeper

1. Chronicity
Firstly, we address the chronicity following a broad approach, including the study of the 7 most prevalent diseases and conditions across primary, secondary and tertiary prevention and treatment. This creates an unprecedented space for data collection and exploitation, produced by several thousand patients in 8 pilot sites.

2. The Platform
The second challenge is the platform which provides the trust authority-enabled secured environment for data acquisition, federation and sharing. The platform is disease-agnostic and provides open connectivity to many different types of sensors, from wellbeing wearables to advanced med-tech devices.

3. Data federation space
The third challenge is the data federation space and AI productivity for early detection, smart intervention and treatments for the different patient’s health conditions. The data federation is indeed the strategic connection of Gatekeeper with the European Health Data Spaces initiative and it will represent one pragmatic implementation of this policy in Europe.

4. Gatekeeper Marketplace
The fourth challenge is the Gatekeeper Marketplace, which is a business transaction ecosystem where stakeholders (e.g., med-techs, industries, SMEs, startups in the supply side, and health care providers, health regional agencies, patients and consumers in the demand side) will supply and procure digital products and data services.

There are many other important issues that will capture the attention of Gatekeeper team such as ethics, privacy, ownership and data protection in the context of data sharing ecosystem for the production of AI based solutions, interoperability with hospital’s legacy information systems, semantics based on W3C’s Web-of-Things standard for assets, resources, devices and applications descriptors and certification of compliance, data model upon HL7-FHIR standard for health care data exchange, clinical use cases definition and experiment design, recruitment, deployment and execution, impact assessment and global evaluation, among others.

For all the above, Gatekeeper is rolling out a strong dissemination campaign producing open-access thematic webinars, where we intend to describe and discuss all the features of the project and the approach to the many challenges and issues.

A challenging but exciting new environment

Soon after the launch of the project, the COVID-19 pandemic erupted in Europe and progressively in the rest of the world. The pandemic has had an important impact on the project execution in all areas. However, we have also discovered and addressed new arising opportunities as the pilot sites adapt to the new circumstances and demands, providing added value to the AI solutions and the experience of the professionals and patients. For example, a new clinical use case was designed specifically addressing COVID-19, and some other use cases were redesigned to cope with COVID-19 conditions.

Even more importantly for the Gatekeeper sustainability plan, is the pragmatic evidence of the benefits and need for remote patient monitoring (RPM) and remote intervention capacity for both COVID and non-COVID patients, anywhere and anytime. RPM aims to become a “commodity” for keeping patients away from hospital’s premises while maintaining the quality of the healthcare service. Here is where the value proposition of the GK platform has its realm: any hospital in Europe and beyond will be able to build a single RPM infrastructure and deliver services for chronic patients, acute conditions, post-surgery follow-up and many more, and create a rich common data space for clinical use and research. Gatekeeper will bring additional added value on top of this.
